•  Maternity options for Flexible Blue Plan 1500: $132.72/mo. Delivery and routine newborn nursery care and pre and post natal maternity care.  Covered 100% after deductible (in-network) and 80% after deductible (out-of-network).  
•  Important:  Maternity benefits are subject to a 180-day waiting period.  If the optional maternity rider coverage is not purchased at the same time as Flexible Blue (i.e., at a later date) the 180-day waiting period for maternity benefits begins with the effective date of the optional maternity rider coverage, not the effective date of Flexible Blue.
